Description / MOA:

PENTORSE 100 contains pentosan polysulfate (not an analgesic). It is a treatment for osteoarthritis that provides pain relief by acting on the pathology within the joints that cause the pain. It also protects and supports the recovery of the joint cartilage that is damaged by the arthritic process. Pentosan polysulfate therefore has been classified as a disease modifying osteo arthritis drug (DMOAD) and represents the rational approach to the medical treatment of osteoarthritis in dogs. It is a sterile pale yellow colour solution, formulated as an aid in the treatment of non-infectious inflammatory joint disease in dogs.

Anti-inflammatory action helps reducing joint inflammation and pain. chondroprotective action and inhibition of proteoglycan helps in terminating further destruction of the joint and initiates regeneration of the damaged tissue. Stimulation of hyaluronic acid increases formation of synovial fluid which helps in lubrication of the joint.

Pharmacology:

Pharmacologically Pentosan Polysulfate is a semi-synthetic polysaccharide polymer. It possesses anti-inflammatory, anti-arthritic and chondroprotective properties. Pantosan alleviates the clinical signs of osteoarthritis by acting on the underlying cause of the disease. Research has shown that it achieves this effect by acting on a number of pathways responsible for the pathogenesis of osteoarthritis.

Interrelationship of pathologic changes in articular cartilage. Subchondral bone, joint synovial lining and synovial fluid that occur in osteoarthritis joints. Once initiated, the interdependent pathways can be self-sustaining, leading eventually to complete loss of joint function. Pentosan polysulfate (PPS) has been shown to act at a number of points in these pathwa some of which are identified in this figure.

Indications for use:

Pentorse or pentosan polysulphate is indicated in the treatment of –

• Osteoarthritis especially in condition affecting multiple joints
• Osteo-chondrosis dessicans
• Traumatic Joint disorders
• Degenerative cartilaginous joint disease
• Musculoskeletal disorders
• Peri-articular inflammation

Dosage & Administration (Canine)

The dosage for canine is 3 mg / Kg BW given intramuscularly or subcutaneously every 5 to 7 days for a period of 4 weeks.

Repeat with a gap of one month.

The course of treatment is to be given 3 times a year depending on the recovery of the dog.

Stimulation of Hyaluronic acid Synthesis – Stimulation of Hyaluronic acid increases formation of synovial fluid which helps in lubrication of the joint and Improves the viscosity of synovial fluids.

Inhibition of Destructive enzymes – Inhibits enzyme implicated in cartilage degradation in osteoarthritis.

Reverses effects of Osteoarthritis – It preserve cartilage integrity and function in a canine model of osteoarthritis.

Fibrinolytic Activity – Promotes breakdown of venous thrombi in the dog which results in improved perfusion of the joint tissue. It normalizes effect on the fibrinolytics systems which is defective in osteoarthritis, thus increasing the perfusion of the joint

Chondroprotective Agent – Chondro-protective action and inhibition of proteoglycans helps in terminating further destruction of the joint and initiates regeneration of the damaged tissue.

Over-dosage / Side effects / Contraindications /Warnings:

It has no reported side effects.

Pentosan being from a plant-based origin doesn’t have any side effects in the body but occasionally if a patient is hypersensitive. They may show gastric sensitivity like Emesis, diarrhea, lethargy, and anorexia.

We also need to take precautions in the following cases:

• Do not use it for the treatment of septic arthritis. In this case, appropriate antimicrobial therapy should be instigated.
• Do not use in dogs with advanced liver or kidney impairment, or evidence of infection.
• Do not use in dogs with blood disorders, coagulation disorders, bleeding or malignancy (especially haemangiosarcoma).
• Pentosan Polysulfate has an anticoagulant effect. Do not use during the peri-operative period.
• Do not use in the skeletally immature dog (i.e. dogs whose long bone growth plates have not closed).
• NSAID and In particular Aspirin should not be used in combination with pentosan as they may affect thrombocyte adhesion and potentiate the anticoagulant activity of the product.
• Do not use corticosteroid as they have been shown to be antagonists to a number of actions of pentosan polysulphate Sodium.

Pharmaceutical precautions / Instructions:

Store at 2°C – 8°C. Do not Freeze. Protect from light. Discard any unused portion of product within 1 month after first broaching the vial.

Safety: Age / Pregnancy/ Withdrawal:

It is completely safe. Do not administer in Pregnant or lactating bitches. Technically the product shows a long term effect as compared to other treatment options.
