Description / Action:
Basmi FIP injection and Capsules contain GS441524. It may also contain Vitamin B12 or Boswellia serrata. GS441524 is a breakdown product of Remdesivir. It is an adenine nucleotide analog that blocks the replication of FIPV (Feline Infectious Peritonitis Virus) by inserting a nonsense adenine in the developing viral RNA.
Half-Life:
It takes approximately 4 hours for the concentration of the drug in the body to decrease by 50%.
Class: Anti-Viral
Indication / Uses:
It is indicated for the treatment of FIP (Feline Infectious Peritonitis)
• Wet FIP
• Dry FIP
• Ocular FIP
• Neurological FIP
• Relapsed cases of FIP
Dosage:
Wet FIP = 6 mg/kg
Dry FIP = 8 mg/kg
Neurological/Ocular FIP = 10 mg/kg
* Only 10 mg/kg and above can break through the blood-brain-barrier
* Recommended relapse injection dosage: 12 mg/kg to 15 mg/kg.
Capsules:
Pink = less than 2.5 kg
Green = 2.5 kg – 4 kg
Blue = greater than 4 kg
Vial Selection:
Above 2.5 kg or Dry/ Ocular/ Neuro: 20mg/ml
Above 4 kg with Dry/ Ocular/ Neuro: 30mg/ml
Dosage Calculation:
Daily dosage = (type of FIP ÷ vial concentration) x heaviest body weight in kg
Weigh your cat once a week to adjust or increase the dosage.
Note 1: dosage should not be reduced to prevent drug resistance
Note 2: the drug only lasts in the body for 24 hours, thus will need to maintain drug presence by administering the drug every 24 hours to prevent the replication of FIPV.
Dosage Required:
Do not reduce the dosage.
Weight increase = dosage increase
Weight decrease = dosage maintain at the highest given dosage
Increase dosage when:
• Severe FIP infection
• Slow recovery progress
• FIP progress to other Forms
For better results provide 2 injections per day each injection using 50% of daily doses amount.
Route of Administration:
Subcutaneous injection (injectables) or orally (oral capsules)
Recommendation:
For sever FIP cases, perform injections at 12 Hours apart instead of 24 Hours until the cat is out of the danger zone. Eg. complete paralysis, seizure, inability to eat at all (dependant on drips).
Injection Zones:
Alternate top, down, left and right or thoracic region. alternate or rotate injection sites to prevent sever skin lesions.
• Avoid the neck – slow absorption and may injure the muscle.
• Avoid hip region as the cat may resist and slow to absorb.
Syringes and Needle to get from local pharmacy
• Syringe 3ml with luer lock
• Needle 21g, 22g (adult cat) or 23g (small kitten)
• Alcohol swab
– Syringe advised to change every 2 weeks
– Needle advised to change daily
Capsule Usage Instructions:
Can be combined with symptomatic treatments.
• Recommended for use after 30 days of injections.
• Provide 1 oral capsule per day according to bodyweight.
(After starting with oral capsules if cat is not showing visible signs of improvement till 5 days you can increase the dosage upto 2 capsules per day or switch to higher concentration formulation/capsule)
• Adhere to a scheduled time.
• Do not skip any day.
• Oral capsules can be taken with supportive supplements such as antibiotics or anti-inflammatory medications.
When NOT to use oral capsules
• When cats exhibit ocular or neurological symptoms.
• When cats suffer from intermittent vomiting or diarrhoea.
• When cats are not eating and require forced-feeding.
How to use:
Provide one capsule per day for up to 84 days. Follow the same timing everyday for best results.
• Insert the capsule into the back of cat’s mouth.
• Squeeze wet cat Snack onto capsule. Cat will eat it without struggle.
• Empty capsule content into wet cat food. Cat should consume all content in one feeding session.
Treatment Duration:
Advised duration of 84 days (12 weeks) to minimise the risk of relapse.
Blood test advised every 30 days to make sure the cat does not have relapse.
Treatment Regimen:
First 30 Days
• Complete blood tests and check parameters before the treatment and after 30 days.
• Provide symptomatic treatments
• Increase dosage if cats demonstrate slow recovery progress.
Next 30 Days
• Complete blood test after 60 days. Check Parameters.
• Continue providing additional symptomatic treatments if needed.
• Increase dosage if cats demonstrate slow recovery.
• If all parameters returned to normal. Stop treatment and monitor.
Last 24 Days
• Blood test at the end of 84 days. Check parameters.
• Is the albumin/globulin ratio at 0.7?
• Continue treatment if parameters are not back to normal.
Observation Period:
First 30 Days
Monitor the following:
• Appetite
• Energy Level
• Nervous System Function
• Litter Activity
If relapse, restart injections immediately
Next 30 Days
Monitor the following:
• Appetite
• Energy Level
• Nervous System Function
• Litter Activity
If relapse, restart injections immediately
Last 24 Days
Monitor the following:
• Appetite
• Energy Level
• Nervous System Function
• Litter Activity
If relapse, restart injections immediately
Recommended relapse injection dosage: 12mg/kg to 15mg/kg.
Shorten parameters check to 2 weeks. Not necessarily to complete another full 84 days.
Overdoses / Side effects /Warnings:
No adverse effect based on study of Dr Pedersen. Clinical trial of GS441524 in the treatment of FIP in cats has shown that there are no adverse side effects after 84 days of treatment. Pedersen noted the “impressive” safety profile of GS441524, with no systemic signs of toxicity observed when administered subcutaneously up to the dose of 10 mg/kg (to treat neurological manifestations of FIP). They discovered that GS441524 treatment produced a long-term cure of neurological FIP with an excellent safety profile, with little dose-related toxicity being noticed, even at these higher levels.
Contraindications:
Antibiotics – Fluoroquinolone
Based on studies, fluorine is claimed to be toxic to the central nervous system (CNS). The cats will then experience neurological symptoms as a result of this. Please be aware that no independent study has been conducted to support this claim. However, based on past cases received by the manufacturer, FIP diagnosed cats who died after receiving treatment with this class of antibiotics. It was approximately 8 cats in total. If a cat still requires the fluoroquinolone class of antibiotics due to some specific sensitive infections then please proceed with care.
Trans-factor (Immune Booster e.g VetriDMG):
Immunosuppression is required instead of boosting the immune system. The use of TF did not increase survivability based on past cases. Also in some cases, it causes the viral activity to increase.
Lysine:
Arginine is crucial for a good, healthy functioning immune system. L-lysine prevents uptake of arginine in the body.
Flea (Spot On):
Flea medication may affect the full effect of GS441524 in the blood.
Use powder form or flea collar instead (external treatment).
